Is it possible to have a new SSS E-1 form?

I lost my original pink copy of E-1 form. My new employer requires me to submit a photocopy of the E-1 form? Is it still possible to acquire new E-1? I believe it is unlawful to have multiple SSS numbers. What will I do? Help me, guys!
